Submitting Profiles to County Office
WHAT
MUST BE SUBMITTED

Student Profile Forms for successful SRA students
2. Copies of their ISRs for each HSPA administration,
and a
3. Signed HSPA SRA School Summary Form (Appendix I)
attached to the top of the student profiles. The School Summary Form lists
information about all of the students for whom a profile is being submitted.
This form should be completed for each school building submitting SRA
student profiles. The School Summary Form also contains the sign-off section
for the school principal and the district superintendent.
SUBMISSION
DATE
Profiles must be submitted to the office of the county
superintendent no later than March 1 of each year (N.J.A.C. 6A:8-5-1[f]1).
